Builder Tab
This is for users to create their own templates. To review how to create and use Subjective and Objective templates, click here.
One of the main difference of the Subjective Template Builder with the Objective Template Builder is the Done For You Sections.
Template Library Tab
This is the list of all created templates. To review how to create and manage templates, click here.
The user has the ability to edit and make changes to their templates except for the Done-For-You sections.
The most commonly used and important feature in the Done-For-You section is the Functional Limitation and Outcome Measure.
In the Library, if the user had chosen a template, it will be highlighted in blue.
And the template with all the questions will appear at the bottom of the template list. The name of the template showing will indicate which template was selected.
The sections that appear on the left are the created sections and Done-For-You sections that were selected during the time the template was created.
Once a section has been selected it is highlighted and turns white. and the questions that are under that section will appear on the right.
When information is carried forward from a completed document, a blue highlight in the section will indicate that there are questions that are answered in that section and that the user must review it prior to saving.
Functional Limitation Notifications
This section is crucial for Medicare patients. The system will automatically prompt you when Functional Limitation is required for the note type you have selected (Initial Evaluation, Progress Note and Re-Evaluation). There will be a pop-up showing the moment you start a new document reminding users to report FLG codes.

In this section you will see a drop down list of different functional limitation to choose from.
Once the user select one of the drop down options, it will automatically provide the Functional Limitation G Codes (FLG codes) associated with the Functional Limitation selected. When the Objective section has been saved, the FLG codes provided will appear in the Claim Review section of the documentation ready for billing.
Supporting Evidence tabs will also appear after selecting an option from the drop down. In each of the tabs once clicked, it will provide a text box allowing the user to type in entries.
IMPORTANT NOTE: The Impairment Rating tab is a necessary field. The Functional Limitation section will not save until the user chooses an option in the Impairment Rating tab. There are 2 areas that needs an entry, the Current and Goal. It will show percentage options that the user can click one option each for Current and Goal.
The FLG Codes that has been provided to the user once the Functional Limitation has been selected, will appear in the claim review tab for submission.
Outcome Measures
This section is crucial for Medicare patients.
In this section, you will see a drop-down list of different Outcome measure questionnaires.
2 Types of Outcome Measures questionnaire
There will be different options provided in the screen depending which selection was chosen.
LEGACY or without Advanced indicated - if this is the option chosen from the outcome measure list, it will direct the user to the partner website orthopaedicscore.com. This allows the patient to fill out the questionnaire prior to the visit
Once you click the link, a pop up will appear leading you to the orthopaedicscore.com.website. Provide this URL to the patient for them to fill out the questions.
At the bottom of the pop up, the patients will have the ability to save the file as a CSV and they can forward this to the clinic.
Once the clinician receives the document from the patient, he has the option to upload the actual test on the patient chart by clicking Upload Test score data. This can be skipped if desired.
But the clinician must enter the test score and click the Save Outcome Measure.
Advanced - there will be outcome measure options that are labeled as advanced. These options are not going to direct the user to the partner website but instead, it will open another window that is still connected within In Touch EMR.
The same as the orthopaedicscore.com screen, whenever the users makes selection on the page, it will start generating a score at the bottom. The user must click save for the score to be added in In Touch EMR documentation.
Unlike the Orthopaedicscore.com questionnaire, the user no longer need to copy and enter the score. Just click on the Display Results button and the score will appear on the documentation right away.
PDF / Word Format - There are some outcome measure options that will have only a PDF and Word document that will provide clinicians how to do the test and properly provide a score. When clicked, it will download a PDF or Word document to the user's computer. This will not automatically provide a score or a questionnaire. The user must do the instructions on the test and enter the score on the EMR page.
